Lire la suite >Depuis 2019, le laboratoire de toxicologie du CHU de Lille met en œuvre en 24/24 un criblage toxicologique par spectrométrie de masse haute résolution (CL-SMHR). Nous rapportons la transformation ponctuelle de cette méthode de criblage en une méthode de dosage adaptée à un suivi toxicocinétique en temps réel d’une intoxication par amlodipine et aténolol. Un patient de 17 ans a été pris en charge par le service de réanimation à la suite de l’ingestion volontaire de 90 comprimés d’ATENOLOL 50 mg et de 90 comprimés d’AMLODIPINE 5 mg. La prise en charge de cette intoxication médicamenteuse sévère nécessitant la réalisation de séances d’hémoperfusion, le laboratoire a modifié en urgence (en moins de 3 heures) sa méthode de criblage pour réaliser des dosages d’amlodipine et d’aténolol en 24/24 : création d’une méthode analytique spécifique pour l’amlodipine [Tr 7,54 min ; (m/z) MH+ 409,1530, fragments 238,0635 et 294,0897] et l’aténolol [Tr 2,92 min ; (m/z) MH+ 267,1709, fragments 145,0653 et 190,0868] et réalisation d’une gamme de calibration pour ces 2 molécules. Ce sont au total 3 séances d’hémoperfusion qui ont finalement été réalisées en 24 heures, guidées par les taux sériques (38 dosages d’aténolol et d’amlodipine ont été réalisés en 24/24 dans un délai moyen de 3 heures) permettant un sevrage progressif des amines et une sortie du patient à J6. Le criblage par CL-SMHR en 24/24 représente probablement l’avenir de la toxicologie analytique d’urgence compte-tenu de ses capacités à constituer un outil adaptable à la quantification en urgence de xénobiotiques, en adéquation avec les contraintes de monitoring clinique.Lire moins >

Lire la suite >Since 2019, the toxicology laboratory of Lille University Hospital has been using high-resolution mass spectrometry (CL-HRMS) for toxicological screening, 24 hours a day. We report the ad hoc transformation of this screening method into a quantification assay method suitable for real-time toxicokinetic monitoring of amlodipine-atenolol poisoning. A 17-year-old patient was admitted to the intensive care unit following voluntary ingestion of 90 tablets of ATENOLOL 50 mg and 90 tablets of AMLODIPINE 5 mg. The management of this severe drug poisoning required repeated hemoperfusion sessions. As a consequence, the laboratory modified its calibration method in less than 3 hours, in order to perform blood amlodipine and atenolol assays repeatedly. A specific analytical method for amlodipine [RT 7.54 min; (m/z) MH+ 409.1530, fragments 238.0635 and 294.0897] and atenolol [RT 2.92 min; (m/z) MH+ 267.1709, fragments 145.0653 and 190.0868] was implanted and calibrated in the analytical system. After analysis of internal quality controls, the first results were returned to clinicians at the end of first hemoperfusion session. Guided by atenolol and amlodipine serum levels obtained from a total of 38 assays, 3 hemoperfusion sessions were performed in the first 24 hours, allowing a progressive withdrawal of amines, and the discharge of the patient on Day 6. CL-HRMS screening might represents the future of emergency analytical toxicology given its capacity to constitute a tool adaptable to the emergency quantification of xenobiotics, in line with the constraints of clinical monitoring.Lire moins >
