Her known medical history included tobacco consumption and epilepsy treated with Lacosamide and Lamotrigine for several years. The first trimester ultrasound examination, performed at thirteen WG, revealed a generalized fetal lymphedema with bilateral pleural effusion, subcutaneous edema and ascites (Fig. 1). A trophoblast biopsy was performed. The karyotype found no abnormality. CMV and parvovirus serology were negative. According to the regional Discussion This case of hydrops with hypoplastic left heart is interesting because of the unexpected evolution after cessation of maternal polysubstance abuse. Because of the unusual improvement, the prognosis and the mechanism were discussed several times. At the beginning of the management, the prognosis of lymphoedema appeared to be poor with a significant risk of fetal death. Then the regression of the hydrops, the hyperechogenic fetal bowel and the hypoplasic left heart evoked a hypoperfusion with Conclusion To our knowledge, this is the first description of a hydrops with hypoplasic left heart improving during pregnancy. The early onset of fetal abnormalities and the favorable cardiac outcome are in favor of transient cocaine toxicity leading to hypoperfusion and ischemia. However a critical aortic stenosis, induced or not, by Lacosamide can not be excluded. Despite the favorable cardiac and hemodynamic progression, the neurodevelopmental prognosis of this child remains uncertain because of the Disclosure of interest The authors declare that they have no competing interest.Show less >
